• PA School Personnel Health Record
• Evidence of a Negative Tuberculosis Test (within 3 months)
• Act 168 Sexual Misconduct/Abuse Disclosure
• Arrest/Conviction Report and Certification Form
• Acceptable forms of ID for I-9 Verification
• Drug Screening
