The Athletic office is now utilizing an online registration platform to register students in grades 7-12 for a sport. This online platform will replace the paper PIAA forms that were previously used. Paper forms are no longer accepted at this time.

If this is your first sport of the year- Please complete the following sections 1-5, 6 & 10. Physicals may only be dated after June 1st of the current school year. 

If this is the 2nd or 3rd sport of the year- Please complete section 7 & 8 (if injured). 

Activity Fee- There is a yearly activity fee to play a sport that is based on your school lunch price. If you are full price $60.00, reduced lunch $30.00, free lunch $0. The fee is due by the first day of practice. Payment can be made at Myschoolbucks without any extra costs, or by paying with a check made out to BHASD or cash.
